I stupidly forgot to ask about the informant details, she didn't read everything out to me, just a few of the fields to make sure she had the right one.

I'll have to get hold of the certificate myself and have a good look at it. I left it with my Mum in the UK (who is on holiday at the moment) so I hope when she's back she can find it and scan it for me. If not I'll re-order it.
