I can only think of a couple of examples that note the explicit reading of banns (in one case the bride + father suffered the non-appearance of the groom!- a couple of Bishops required that the minister note as to whether by banns or licence + by whom issued (eg there is a directive by Bishop Wilson) - also the early Malew registers pre 1690 have B or L against marriages.
I think the licences were relatively cheap (certainly as compared with English practice) and my estimate is that about 90% married by licence
