I think you'll find Eleanor was Shimmin nee Shimmin - not unusual.

Patrick & German are adjacent parishes. Depending on the exact location, it is not unusual to be born in one parish but baptized in an adjacent one.

As far as census data are concerned, you can never be absoulutely certain whether "birth place" is actually place of birth or place of baptism.
