My understanding of patrinus/matrina fuit is slightly different.

In the RC baptisms I've seen it's been like this:

[date] nata (=born) , [date] baptisma (=baptised)
[child's name, Latinised] filius (=son) or filia (daughter) [of father's name] et (= and) [of mother's name] olim (previously) [maiden name]

patrinus fuit (= godfather was)
matrina fuit (= godmother was)

In the marriages, "de" is intended to mean "of", as in where they lived, and "filium" or "filiam" meaning "son" or "daughter" (of) follows the groom or bride's name.
